1 · How does the failure behave over time? (Framework A)
2 · Is the failure evident to operations?

Under normal conditions, does anyone notice the failure occurred (alarm, visible loss of function)? Protections and redundancies are usually hidden.

3 · Is there a measurable warning before failure (P-F interval)?

Vibration, temperature, thickness, noise, process trend — something that leaves time to act between the signal (P) and functional failure (F).

4 · What is the worst plausible consequence?
